The Case Management Policy Specialist, as part of the Disability Services Division (DSD) will develop, implement and assess statewide case management policies and procedures for people with disabilities receiving home and community-based services through:

  • Community Access for Disability Inclusion (CADI)

  • Community Alternative Care (CAC), Brain Injury (BI)

  • Developmental Disability (DD) waivers

  • Developmental Disability Rule 185 Case Management

  • Vulnerable Adult and Developmental Disability Targeted Case Management

Responsibilities include:

  • Develop and implement case management policy for individuals with disabilities in accordance with Minnesota statute and federal regulations

  • Develop, deliver, and maintain DSD case management technical assistance and training to improve the competency of disability case managers

  • Establish, conduct, and monitor policies, quality assurance activities, and standards of performance to ensure lead agencies and case management providers deliver equitable, integrated case management services

  • Support statewide systems design and maintenance for use by case managers

  • Foster an engaging and professional environment committed to respect, inclusivity, continuous improvement, and teamwork

Minimum Qualifications

3 years professional experience in Home and community-based long-term services and supports (LTSS) for people with disabilities. Experience must include:

  • In-depth knowledge of federal and state laws and regulations related to Minnesota's Medical Assistance program, including federal and state Medical Assistance waivers

  • Comprehensive understanding of Minnesota case management systems and processes, particularly in relation to home and community-based long-term services and supports (LTSS) for individuals with disabilities

  • Familiarity with the roles and relationships among county, tribal nation, case management providers, and state and federal agencies, ensuring inclusive and effective collaboration across all levels

Bachelor’s Degree in social work, public health, public policy or human services may substitute for 1 year of experience

Preferred Qualifications

  • Proficiency with the MnCHOICES application, including experience in utilizing the tool for assessment and case management processes.

  • Familiarity with the Medicaid Management Information System (MMIS), including experience in data entry, retrieval, and analysis within the system

  • Experience in integrating, coordinating, and reconciling the interests, mandates, responsibilities, goals, and perspectives of diverse stakeholders across agencies and disciplines

  • Demonstrated ability to work effectively with diverse communities, including individuals with disabilities, in roles such as conducting community outreach, providing culturally responsive services, and facilitating communication and collaboration across different cultural backgrounds and languages
