Summary

This is a 1.0 FTE with the Addiction Consult Team (ACT). The peer will be part of an interdisciplinary team which sees patients across the Michigan Medicine emergency departments and inpatient medical floors. The team provides assessment, development of treatment plans, utilizing evidence-based treatments (including medications for opioid use disorder), and ensure a robust warm handoff to ongoing outpatient care/follow-up, either at Michigan Medicine or within our community. The team has a long-term goal of empowering and upskilling our care teams across Michigan Medicine to care for patients with substance use disorders just as they would care for patients with any other chronic disease.

Responsibilities*

You will be responsible for:

  • Engaging patients to obtain background information and assess readiness for treatment.

  • Providing psychoeducation about substance use disorders.

  • Providing discharge planning for patients with substance use disorders, including coordinating and facilitating post-acute transitions where needed

  • Providing education as needed to empower and upskill our care teams across Michigan Medicine to care for patients with substance use disorders

  • Identify difficulties in transitions of care for substance use disorders and advocate for solutions within Michigan Medicine

  • Participating in team meetings and other care coordination meetings on the ACT service.

  • Involvement in community engagement through networking to build and sustain relationships.

  • Supports patients through the ability to share parts of peer recovery journey/story.

Required Qualifications*

  • Direct lived experience in substance use disorder and successfully living in active recovery (i.e., either in abstinence or stable on FDA approved medication for substance use disorder).

  • Two years of experience in the recovery coaching field including working as part of a diverse team and open to ongoing learning and feedback.

  • Certification in the field of addiction and/or recovery coaching or working towards certification obtained within one year of hire/transfer date.

  • Demonstrated skills/experience in motivational interviewing (MI) techniques, and completion (or working towards completion) of an MI training program.

  • Comfort with utilizing harm reduction models of care

Desired Qualifications*

  • Experience working with the recovery community in SE Michigan.

  • Experience engaging with vulnerable populations.

  • Experience with presenting to various disciplines including social workers, nurses, physicians, students, and other trainees

  • Previous experience in a medical setting

  • Skilled in developing rapport and effectively communicating with a diverse patient population.

  • Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic.

  • Independent, sound decision-making is necessary for success in this position.

  • Effective collaboration with a large multi-disciplinary team.

  • Proficiency in keeping accurate and timely clinical records by computer.

Work Schedule

This is a 1.0 FTE position; daytime hours (8am-4pm or 9am-5pm)
