Job Description:
Duties/Responsibilities:
Assess physical, mental, and psychosocial status of patient and review barriers for enrollment/competition of Cardiac and Pulmonary Rehab (example: financial, transportation ect…).
Plans, evaluates and implements care of patients in the Cardiac and Pulmonary Rehab program as a member of the interdisciplinary team as is indicated in the initial and ongoing assessment.
Participates in the planning and delivery of formal and informal educational programs for all patients, family members and the community as is appropriate.
Consult on in-patients that have been admitted for a cardiac and/or pulmonary event to provide information on the program.
Work closely with Cardiac Rehab Support Staff on transitioning patient from in-patient stay to first appointment in Cardiac or Pulmonary rehab.
Use patient tracking form to ensure we are meeting quality metrics on adherence and discharge to enrollment in Cardiac rehab and Pulmonary Rehab.
Documents information clearly and completely, utilizing the correct format in charting.
Assists with planning, implementing and maintaining QA& I activities and monitors.
Ensures that equipment and supplies are available.
Ensures that environment is safe, clean and comfortable.
Prepares area and patients for exercise.
Work practice reflects philosophy, purpose and policies of the hospital and the department of Cardiac and Pulmonary Rehab.
Act as the clinical lead for clinical emergencies and works closely with the Clinical Exercise Physiologist on patient triaging at the start of each appointment.
Performs new patient evaluations including but not limited to medication reconciliation and accurate medical history intake
Assists with clerical duties.

Education/Experience Required:
Graduate of an accredited school of nursing
B.S.N. required per DPH regulations
Minimum of 2 years critical care experience or 4 years of cardiac nursing experience required
Experience working in Cardiac Rehab and Pulmonary Rehab preferred specifically performing the following duties:
performing and/or interpreting cardiac and pulmonary stress tests
writing and interpreting exercise prescriptions
monitoring and interpreting hemodynamic responses to exercise
Advanced EKG telemetry monitoring and interpretation
Experience in planning and delivering formal educational programs on medication compliance, symptom management, nutrition, and exercise on patients with congestive heart failure, cardiac disease valve abnormalities, and pulmonary disease
Experience in nursing leadership ie charge nurse required
Demonstrates strong computer skills, specifically with excel, documentation, and use of electronic medical record
BLS and ACLS certification required
Experience with case management and/or social work preferred
Registration/Certification:
Current RN Massachusetts License
Current ACLS & BLS from the American Heart Association
AACVPR Certification required within 1 year of hire
Wellness Coaching Certification desired
Diabetes Education Certification desired
Physical/Environmental Requirements
This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.
